Dental, Life Insurance and Long Term Disability
Guardian will continue to be our insurance company in 2019. There will be no rate changes in any of the plans.
Dental Open Enrollment is held in November. Employees can enroll for the first time or add dependents to existing coverage. The deadline is Monday, December 3, 2018. Coverage will be effective 1/1/19.
Dental enrollment forms can be faxed, scanned and emailed or mailed. Texas residents please use Texas Residents Only form (PDF). Please email InsurancePlans [at] uua [dot] org to request a secure email link before emailing forms.
There is no open enrollment period for Life and LTD Insurance.
What's New in Life, Dental and Long Term Disability For 2019?
DENTAL INSURANCE: STABLE RATES, STABLE INSURANCE COMPANY
Guardian will remain our insurer for 2019, and the rates will remain unchanged at Individual: $56 per month; Two-Person: $112 per month; Family: $137 per month. We have made a small adjustment to the deductible, increasing it to $75/individual, $225/family.
Open Enrollment is the once-a-year opportunity for current employees to enroll in the UUA Dental Plan and to add dependents without a qualifying event.
LIFE INSURANCE AND LONG TERM DISABILITY INSURANCE
Life Insurance and Long Term Disability (“LTD”) Insurance will remain with Guardian for 2019. Guardian’s service has been excellent, and their rates remain competitive. There is no Open Enrollment Period for either Life or LTD.
For 2019, the rates will remain unchanged, and there are minor benefit changes to protect our pricing and keep the plans in line with the market.
Long Term Disability: The annual cost is 1.2% of annual salary (salary+housing for clergy). The benefit will change from 66 2/3% to an industry-standard 60%.
Life and AD&D: The monthly cost is $0.34 per $1,000 of insurance. The benefit remains 2 X annual salary (salary+housing for clergy). We have eliminated two rarely used benefits: premium waiver and portability. A conversion option remains in place. The benefit will be reduced to 60% of the pre-65 level at age 65, and to 35% at age 70.

- Be sure to report all employee changes affecting insurance—including terminations, income, addresses, and qualifying life events—to the insurance_plans [at] uua [dot] org (Insurance Plans staff) in the Office of Church Staff Finances.
