Important information about Insurance Plans billing for those working offsite: if you cannot access your mail please email firstname.lastname@example.org to request a copy of your monthly invoice. Our bank lockbox can only accept checks. It is permissible to use a bank bill payment service to issue the check. Please make sure your account number is on the check. The remittance slip from your invoice is not required.
2021 Insurance Premiums
In this time of uncertainty, we have negotiated renewals for our Dental, Life, and Long-Term Disability plans that will keep 2020 rates and benefits unchanged for all of calendar year 2021.
For the Health Plan, benefits remain unchanged and there will be no base rate change on January 1. We are keeping open the possibility of a rate adjustment in July, however. Because rates are based on age, some Health Plan participants will experience a small age adjustment as we move into 2021.
We offer high-quality insurance plans that express our Unitarian Universalist values. We hope that this unprecedented rate and benefit extension helps you with your budgets.
Open Enrollment for Health and Dental
As always, November is Open Enrollment Month. Open Enrollment is the once-a-year opportunity for current employees to enroll in the UUA Health Plan, if they had missed an earlier opportunity, or make upgrades to their plan choices, and to add dependents without a qualifying event.
For Dental, it is the once a year opportunity to join the Guardian Dental Plan if they had missed an earlier chance, and to add dependents without a qualifying event.
No action is required for employees not making changes.

Guardian Dental and Vision Benefits
If you elect to enroll in Guardian Dental, vision benefits are covered through EyeMed at no extra charge to everyone enrolled in the Guardian Dental Plan. We are offering Dental and Eyewear as a package, as most large employers do, but keep in mind that the companies are separate—Guardian does Dental; EyeMed does eyewear. You will receive an EyeMed ID card in the mail directly from EyeMed if you are enrolled in the Guardian Dental Plan.

Life and Long-term Disability Plans
There is no Open Enrollment Period for either Life or LTD, so it is important that you continue to enroll staff in these very important benefits within 60 days of their first eligibility. Late enrollment can be complicated, and will require medical evidence of insurability, which can be difficult to meet.
Life Insurance and Long Term Disability (“LTD”) Insurance will remain with Guardian for 2021. Guardian’s service has been excellent, and their rates remain competitive.
Long Term Disability: The annual cost is 1.3% of annual salary (salary+housing for clergy).
Life and AD&D: The monthly cost is $0.35 per $1,000 of insurance. The benefit remains 2 X annual salary (salary+housing for clergy).
