Important Plan Administration Reminders

For Congregational Employers and Administrators

  • Use our new Benefit Enrollment Form to enroll new employees
  • Use our Employee Benefits Change Form to:
    • Terminate employees’ coverage
    • Change or update name, address, or other personal information
    • Update employees’ salary
    • Report any changes in employee benefits
    • Report a transition from one Congregation or UUA affiliated organization to another
  • All employee changes must be submitted within 30 days of the effective change. We’re unable to process refunds or credits that go back more than 60 days from the date of the change.
