Thrive Young Adult Forms

Every Thrive Young Adult attendee must fill out several forms and submit them as soon as possible.

Now, this process can be tricky, and the deadline is pretty tight, so if it's a challenge that's understandable. Just email or call Deborah at youth@uua.org or (617) 948-4355 right away and she'll help you figure it out so you can meet the deadline.

Forms to Fill Out

  3. CONSENT AND RELEASE FORM: Online -OR- PDF

Submitting Online Versions

Once you hit the blue "DONE" button on forms #2 and #3, it is automatically submitted to us and you don't have to do anything else. Your work is done!

Submitting PDF Versions

If you have chosen to fill out hard-copy PDFs, please print out, fill out, and sign forms #2 and #3 (there is no PDF version for #1; just email us with your information).

Please send them to us using ONE of the following secure methods:

  1. FAX: 617-948-6473
  2. SECURE EMAIL SYSTEM: DO NOT EMAIL your PDF form to a UUA.org email address—the UUA email system is not secure enough to safeguard the sensitive personal information on these forms. Email Deborah at youth@uua.org and she will send you an invitation to join our secure email system, through which you can email electronic versions of your forms (scan or a very bright clear photo).

We ask that you DO NOT email these forms to any UUA.org email address, as the UUA email system is not secure enough to safeguard the sensitive personal information on the forms.
