UUA Congregations Theme is incompatible with Gravity Forms

This is an informational post just in case someone else is trying to use Gravity Forms with the UUA theme. Something in the theme is not compatible with Gravity Forms (http://www.gravityforms.com/). As soon as I try to use conditional logic on any field in the form, the form disappears. I reported this to Gravity Forms and got this reply: Because your form uses conditional logic it has an inline style of display:none applied, if your page has any JavaScript issues they can prevent the Gravity Forms scripts from running, and when those scripts can't run the inline display:none style can't be removed. I temporarily switched themes and my form came back. I also tested deactivating all the plugins with the UUA theme, but the problem was still there. Gravity Forms is a paid license tool that I have used on several other websites and I like it for the conditional logic and PayPal integration. I want to use Gravity Forms for my Donate page, here is how it looks on my old website (http://uufscc.org/wordpress/donate-2/). The work-around that I will use will be to create a one page website in a sub-domain using a different theme and link the donate/giving in my main site to this other site.
