Dental Insurance
We are pleased to provide dental coverage through Guardian until December 31, 2025. Starting January 1, 2026, our dental plan will transition to Delta Dental, one of the nation’s largest and most trusted dental networks. Delta Dental offers broad access to dentists, strong coverage for preventive care, and easy-to-use online tools for managing your benefits. If you are currently enrolled in dental coverage through Guardian, your enrollment will automatically transfer to Delta Dental, and you will receive new ID cards before the start of the new year.
Premiums
- Individual: $65 per month
- Two-Person: $130 per month
- Family: $150 per month

Enrollment
You have 60 days from your date of hire to enroll in Dental insurance. Please see our page about qualifying events for enrolling outside that time period. You may also enroll during Open Enrollment, held each November, with coverage effective on January 1 of the following year.

Employee Assistance Plan
Guardian also provides an Employee Assistance Plan to all members enrolled in the life, LTD and/or dental plans. Help with Health, Family, Legal and Financial issues is available online at Uprise Health Employee Assistance and Mental Health and by phone at 1-800-386-7055. The website contains an abundance of helpful information including articles, videos, classes and more. Help is also available by phone and by referral to legal, counseling and other providers.
Vision Plan
If you enroll in Dental, vision benefits are covered through EyeMed at no extra charge. Keep in mind that the companies are separate—Delta does Dental; EyeMed does Vision.
EyeMed no longer sends physical ID cards as a part of their Go Green initiative. You will need to create a member account to access PDF copies of your ID cards.
