Renaissance Program Participant Evaluation Form

Please submit the Renaissance Program Participant Evaluation Form within one week of completion of the Module; Renaissance certificate will be sent within ten days of receipt of evaluation. All feedback is confidential and is seen only by Renaissance staff; feedback to leaders is shared only in aggregate. Your candid comments are very helpful in developing strong leaders and a strong Renaissance program. Address any questions to renaissance @ uua.org.
