Renaissance Program Leader Evaluation Form

Please submit the Renaissance Program Leader Evaluation Form within one week of completion of the Module. All feedback is confidential and is seen only by Renaissance staff; feedback to leaders is shared only in aggregate. Your candid comments are very helpful in developing strong leaders and a strong Renaissance program.

Comments regarding registration, logistics or meeting space should be address to the module coordinator from the sponsoring group.
