Congregational Administrator Guide
This page provides step-by-step instructions and resources for managing employee health plan enrollment and related responsibilities.
Enrolling Employees
If your congregation participates in the UUA Health Plan and has a Subscription Agreement on file, all eligible employees, those who work at least 15 hours per week and receive a W-2, must be offered coverage. To enroll employees:
- Make sure your congregation has a Subscription Agreement on file by emailing insuranceplans@uua.org. Employee enrollments can begin as soon as the Subscription Agreement has been submitted.
- Access the online Enrollment Application.
- Complete the required employee information.
- Submit the online application through JotForm.
- Confirm enrollment with the employee and maintain confirmation e-mail for your records.
Updating Employee Information
To update an employee’s coverage or personal information:
- Complete the Benefit Change Form via JotForm.
- Submit the form online through JotForm.
- Once successfully submitted, you will receive a confirmation email. Keep a copy of this e-mail for your records.
This ensures that any changes, such as adding dependents, changing coverage level, coverage amounts, or updating contact information, are accurately reflected.
Annual Required Splits Form
Each year, congregations must submit the Annual Required Splits Form via JotForm online by December 31st. This form documents:
- Employee and dependent premium contributions for the UUA Health plan only.
- Do NOT include any information for Life, Dental or Disability payments.
ERISA rules require that employers track, and that the UUA report separately, the amount of UUA Health Plan premium funded by the employer and the amount paid by employees for themselves and their dependents.
Compensation Standards
Congregations are not required to contribute the same percentage of premium for all employees, but it is recommended to review the UUA Compensation Standards for guidance on:
- Recommended contribution levels for employee coverage
- Equitable treatment of employees across positions
- Best practices for benefits administration
