Premiums for the UUA Health Plan

Unitarian Universalist Association (UUA) Health Plan Premiums are based on:

Plan Choice
- Standard PPO
- High Deductible PPO (with or without an eligible Health Savings Account)
- Medicare Supplemental
Employee's Age
Employee's Home Zip Code
Coverage Level:
- employee only
- employee plus spouse or partner
- employee plus child(ren)
- family

Dependent Premium Calculation

Many congregations choose to contribute separate percentages for employee and dependent premiums. To determine dependent premium:

Calculate total monthly premium for coverage level (family, employee plus spouse or partner, employee plus children) using ourRate Calculator
Calculate total monthly premium at the individual coverage level for employee
Total Premium—Individual Premium = Dependent Premium
