Employee Premium Calculation

Unitarian Universalist Association (UUA) Health Plan Premiums are based on:

• Plan Choices
• Employee’s Age
• Employee’s Home Zip Code
• Coverage Level:
  • Employee Only
  • Employee Plus Spouse or Partner
  • Employee Plus Child(ren)
  • Family

Dependent Premium Calculation

Some congregations choose to contribute different percentages for employee and dependent premiums. To calculate the dependent premium:

1. Use the Rate Calculator to find the total monthly premium for the desired coverage level (family, employee + spouse/partner, or employee + children).
2. Use the Rate Calculator to find the total monthly premium for individual (employee-only) coverage.
3. Subtract the individual premium from the total premium:

Total Premium − Individual Premium = Dependent Premium
