Five Wishes - Planning for Advanced Care

The Five Wishes program and planning booklets are widely used by Unitarian Universalist ministers and families to help facilitate conversations about end-of-life issues and advanced planning that addresses spiritual and emotional needs as well as medical and legal concerns.
The Five Wishes website offers resources (and group discounts for their planning booklet) that you can use in a congregation-based workshop.
Another option is The Conversation Project, which offers free guides for end-of-life conversations.
