Forms
Design
Web forms shall use the following consistent formatting:
- Use class="form" on the table tag. Do not specify width, padding, or spacing.
- Table has 2 columns, colspanned for textareas and header cells. If the form has required fields, use "*" to indicate a required field.
- Header cells should use the <th> tag instead of <td>.
- Label cells should use the class="form_label".
- Input cells should use the class="form_field".
- Textareas have 5 rows and 45 columns, and should be placed in a table cell that spans the entire table.
Script
We use the formmail script, which can be called using the following code:

Our scripts require the "email" and "realname" fields in order for mail to be delivered to your recipient.
Security
Forms that ask users to submit sensitive or confidential information should use our secure server.
Links to secure forms must be treated as external links in the following format:
https://secure2.uua.org/FORM_URL
There is no way to add an external link to our sidebar navigation. Therefore, all sensitive forms should include the following note:
If your browser is not showing this page as a secure site, please switch to our secure server before completing this form.
Where "secure site" links to information about how the user can ensure browser security, and "switch to our secure server" contains the link to the form using the secure format.
