Stanfield and D'Orlando Scholarship Application Checklist

The following must be received by February 15:

Completed Application Form (Word, 8 pages; preferred format) (PDF):

• Personal Information (Pages 1-3 of application)
• Essay (1-2 page essay as outlined on page 3)
• Personal Financial Statement. (Pages 4-5 of application)
• Personal Tax Information (Copy of most recent tax return or equivalent)
• Parent/ Spouse Financial Statement (Pages 6-7 of application)
• Parent/ Spouse Tax Information (Copy of most recent tax return or equivalent)
• Slide Portfolio (Art applicants only)
• List of Works (Art applicants only)

The following supporting documentation must be received by March 1:

• Transcripts (Official transcripts from all institutions attended)
• Recommendation from Educator (Dated letter from current instructor or teacher)
• Recommendation from Church (Dated letter from your minister/ officer of the Church)
