Forms

Design

Web forms shall use the following consistent formatting:

  • No padding/spacing specified on the table.
  • Table has 2 columns, colspanned for textareas and header cells. If the form has required fields, use 3 columns with the center column for the "*" that indicates a required field.
  • Header cells are left-aligned, and start with a &nbsp; <br /> between sections.
  • Field label columns are right-aligned, and ideally should be brief, so there's no line wrapping.
  • No sizes specified for regular text fields, and all regular text fields should be aligned by placement in the right-hand column.
  • Textareas have 5 rows and 45 columns, and should be placed in a table cell that spans the entire table.

Script

We use the formmail script, which can be called using the following code:

<form action="https://secure2.uua.org/cgi-bin/secure_formmail.pl" method="post">
